Trochanteric Hip Bursitis Symptoms

"Greater trochanteric pain syndrome affects about 1 in 300 people each year. It is most common in women between 40-60 years of age. It can occur in younger people, especially runners, footballers and dancers."

As you probably know, trochanteric bursitis causes pain in the outer areas of the upper thighs, as can be seen in the image just below. If the condition becomes severe enough, the patient will have difficulty walking.

Trochanteric bursitis pain pattern.

A healthy greater trochanteric bursa is usually about the size of a small plum, but can swell to the size of an orange when bursitis sets in. Other symptoms you may experience include:

  • Pain around the bursa that may extend down the groin and thigh
  • Swelling and tenderness in the area of the bursa
  • Pain will likely increase when the hip is flexed or activities such as climbing stairs or running are undertaken
  • Difficulty sleeping due to pain. Sleeping on the affected side will likely aggravate the pain.
  • Limping
  • The area around the bursa is hot to the touch
  • Stiffness and tightness in the hip joint, groin and possibly the knee due to a tight muscle (iliopsoas) or tendon (IT band)

General Trochanteric Bursitis Breakdown

Relieving the symptoms of bursitis initially focuses on keeping the pressure off the bursa and resting your hip. This can be difficult when you have to carry on with daily activities, but resting whenever you can is recommended. During your recovery you will probably have to modify or avoid the activities that stress your bursa until your pain and inflammation settle. Most cases of trochanteric bursitis eventually go away on their own within 1 to 3 months.

Treatments should involve decreasing swelling, relieving stress on the hip joint, correcting any bio-mechanical dysfunction (uneven leg length, bone spur, etc.), treating scar tissue, and then restoring strength and movement in your leg and hip. Surgery may be required if your bursa irritation is a result of a bone formation problem, such as a bone spur.

Should you seek medical attention?

Seeking medical attention

This is up to your discretion; however any continued discomfort in your groin or pelvis area should be investigated. If you continue to experience the iliopsoas tendinitis or bursitis symptoms and have tried the suggested conservative treatments (see conservative treatments for iliopsoas tendinitis or bursitis), it is recommended that you seek professional medical attention. If you experience any of the symptoms noted below it is recommended that you seek immediate attention:

  • Severe pain and tenderness.
  • Problems or swelling in or around the genitalia (penis, scrotum, testicles).
  • A cut, lump or bulge or bleeding in your groin area.
  • Major hip/thigh movement problems causing a severe limp.
  • Urinary problem.
  • A groin rash.
  • Postoperative problem after groin operation.
  • Exposure to an STD.
  • Groin pain has not improved after 1 week, where symptoms are more severe or frequent.
  • Signs of shock (light headedness, restlessness, shallow breathing, sweating, weakness, nausea.)
